Is this hith or just a blemish? He does a lot of digging and ramming, but it looks a bit deeper than a normal "missing scale".

Nitrate test today however revealed:

Looks like an injury to me - probably hit a rock or the lid too hard.

Could very well be HITH. White lesions such as this can indeed be HITH. But it looks like early stages. I would keep an eye on it and make sure you are doing frequent large volume water changes.
When testing for Nitrates make sure you shake the testing solution a LOT before putting in the vial.
Some say high Ph can lead to this. Some say this occurs after the oscar is 3 to 4 years old. I had an oscar that had white lesions such as this. Same area of the head. Not good.

He's not an oscar, but I get what you mean. I was surprised by the low nitrate count too, that's why I ran it twice. I'll definitely be keeping an eye on him though. Hopefully it's just an injury and it heals.
