Hole in head on teugelsi bichir ?

I looked at my teugelsi today and noticed this very strange white stuff on his head. It looks like someone bit him but he is not missing any skin or those nostril straws. It doesn't look fuzzy or anything.Its almost like it got bleached white in certain areas and none of my other bichirs have it. Any help would be extremely helpful thanks.

tank size, stock, water change schedule, water perams and filtration?
 
Looks normal to me. It could be rubbing against the glass. Some bichirs "whiten" around that area after some time. My biggest concern are those eyes. Don't know if it's the picture itself but the eyes seem to have some pretty serious cloudy eye usually caused by high nitrates/bad water parameters. Do large water changes often & add some aquarium salt. Best of luck.

100 gallon, 30% water change 3x a month, the tank itself is beyond overstocked but I have a 210 gallon complete except for some additional filtration(any ideas for the best bang for your buck filter?) I have a fluval 405, built in sump. I have had a problem with ammonia spikes recently but no problems with nitrite which strikes me as odd. There are other bichirs none with this problem. His eyes are fine it's just the picture.
 
but, and what you are going to do does not help your fish now. again fish size and how many. test kit being used.

if you know your tank is over stocked why only doing 30% water change 3 times a month, should be doing 80% or fin level at least every week. ammonia in water means you are badly under filtered and the fish may be ammonia burned.
not trying or wanting to be an ass here. but not going to sugar coat either.
