is she suffering from "hole in the head??"

unless the dip is broken open flesh I'd say it's skinny. That hollow between the eyes is a classic sign of a underfed ray.
I've not heard of rays getting HITH.

it looks like a healthy ray other than it needs to be fed a few times a day for awhile till it fills in. I have confidence in any ray after what rook made happen!
