Hole in the head?

it looks like a pimply, warty thing, about 2mm poking out the head.

One of my Oscars had something that looked like that when it was young, I never did find out what it was for sure but i figure it was a cut that got infected as he loves to dig and would constanly bang his head on the decor and then dig through the gravel.
I just uped the water change to 25-35% every 2 days and it cleared up and has not come back since, that was about 6 -8 months ago.

how big was he when this happened? my texas is about 8" long

ok. Well i'm doing 25% daily water changes and I've added salt and anti fungal med. we'll see how he goes. he seems to have scraped of the warty thing, it's just a cut sitting on his head

Doesn't look like heximita to me. HITH looks more 'errosive' in appearance
