How did my oscar get hole in the head?

I've had a tiger oscar for about six months. He's about 5 1/2" long. I think he may have hole in the head. How is this possible? I keep my water pristine. Ammonia is non-existant, Nitrate barely registers, ph is 6.9. I feed him Hikari Cichlid Gold. What could have caused this?

Possibly boring diet. Not getting ALL the nutrients needed. Feed about 2 other different foods, (recommend another hikari with different vitamins), and another pellet or freeze dried. Freeze dried krill would probably work best.
Then beefheart once or twice a week.

Another possibility is stray electricity in your tank.

Do you clean filter media often? If not there is a lot of waste building up, and there is too much beneficial bacteria. They then absorb too many vitamins and nutrients and your oscar isn't receiving enough.
 
There's no way to tell... You could have stray current. Or you could have too much BB. Or you could have a boring. Or you could have one of the other possible causes... All you can do is try one at a time.
Try to mix up the diet a lot. Like cichlid gold in the morning, krill at night, then beefheart once a week.

Then clean out your filter media of waste. This should help lower BB counts to what you actually need.

If that fails try stray current. You need to ground your tank then find out where the current is coming from (heater or powerhead most likely).
 
what sized tank is your 5.5" Oscar in? If it feels cramped it may be stress related. BlueHairMan brought up some points I wouldn't have thought of but are possible causes...
 
He can't be cramped. He's in a 180 gal with similarly sized second oscar 2 firemouths and a plec. The two bio-wheels and filter pads in the sump haven't been cleaned in three months. Would Seachem Stress Gaurd (promotes healing) help at all, assuming I solve the source of the problem?

well, what he is going to say is that the use of carbon can contribute to HITH. it is debatable just like the other 15 possible causes. best bet is to try your best to solve all possible issues, and hope for the best. good luck!
