he is drawing the snake's attention to his wiggling foot and hand and leg gestures.
Cobras cant really attack upwards either.

so while the snake is pre-occupied with the guy's foot, he sneaks a kiss over its head where it cant really get him anyways.
Of course, these people learn to "read" the snakes to know when they are likely to strike.
