How to Make A Tank Escape Proof

Over the years before I became a member on here, I had trouble with fish jumping out of the back of my tanks. I had been rid of this problem for a while up until a few weeks ago. So I wanted to ask how do you guys make sure that your tanks are secure and escape proof from fish? Excluding big canopies, or anything like that because I am looking for something that I can modify for my tanks. Anyway in anticipation, thank you.
 
I use clear plastic vinyl sheets from folders and cut it to shape then use tape to cover up the holes.

From a house of 8 moray eels i can tell u the thing that has worked here is black duct tape on gaps on the back and make sure it covers every inch. You hve to tape the filters so they connect with the hood. Little tape on lid and its worked for us for years.

Thanks guys I think Im going to go with the duct tape idea because it sounds simple, cheap, and effective.
 
I always go with eggcrate and cover the back and weight it or tape it down.
