MY flowerhorn is lying on his side at the bottom of the tank and difficulty of breathing. White poop is in his anus and a very severe duck lip I ever saw. I dont know if I should blame myself, I mean i always do feed them twice a day and 40% water change every 5 days. My FH got sick last 2 weeks and at that day I already did a 50% water change everyday with continous meds and salt. I regulated the temp and PH of the water. I even used 3 kinds of meds with complete regimen. I put an aerator beside him but still his not cured. I already did everything. It proves that even me as a 11 years of experience in fish keeping could still experience a tragic death. He is now very very weak.

---- I think he wont survive for the next 24 hours ----

:(

What meds did you use? Did you combine them or use them on different intervals? I notice a trend in which some people would combine several meds altogether. This will only worsen the situation than help. A lot of meds create a very potent chemical reaction when combined. Could you please tell us how you treated your fish?

How did you deal with your pH? I am suspicious if you are fiddling with your pH too much.
How about the temp?

At first, I thought it was only a simple bacterial infection, so I did a 50% water change and add 1 tbs of salt per gallon then methylene blue 5 drops per gallon. I did this for 4 days and their is no improvement. On the 5th day, I already raised the temp to 79 fahrenheit because I already knew that this is a severe case because he is already not eating and not moving with white poops in his anus. I tried Mebendazole with malachite green for 4 days with again 50% water change and salt. Their is still no improvement, I already moved him in to my quarantined cycled tank with very similar ph water from where he came 6.2 with no meds because I now worry of him getting a secondary illness like ammonia burn and stress from meds... He is currently under my observation in a 15 gal tank with an aerator beside him... Hoping for the best...

If it were me, the only course of action I would do is add a teaspoon per gallon of salt every 12 hours within 24 hours until you reach two teaspoons per gallon total. You were using tablespoon, right? It's a huge jump equivalent to three teaspoons per gallon in your case. Mebendazole can be included however neither MB nor MG should be used. Metronidazole is your other choice. Allow the salt to take its course of action in disinfecting any infections as it does prevent bacteria from latching on any open cuts, wounds, etc.

Do you have results for ammonia, nitrite and nitrate? What is your pH usually? 6.2? What test kit were you using? Ammonia though is not easily an issue where low pH is concerned as it would be converted to harmless ammonium. If your pH seems to swing, place a bowl of crushed corals or seashells in your tank to minimize pH swings.

yah I forgot to mention, I only put salt on half the tanks volume because I did a 50% water change. Just to make things clarify. I also didnt go over for metronidazole because its much more teratogenic than the mebendazole. I red this from a scientific study. Yes you were right about the ph swings, I think its because of frequent water changing and meds. I use chemical testing kits for labs, thought that it would be useful in monitoring the ph...

How low are the three? Get crushed corals to help stabilize the pH (even if assuming the test may not be accurate).

Just my opinion, but before you started adding salt and meds, why not try figuring out exactly what the problem is? I guess it's too late for that now... Second... You raised your temp to 79? My FH has his tank at 80-82 constantly.

Seeing as he probably won't make it, I would suggest euthanizing the fish vs. letting him suffer.
