36 gallon bow front
3" Oscar, 3" synspilum, 3" chocolate cichlid, and 3" red spot Severum.
Parameters Ammonia: 0, nitrite: 0, nitrate >20, temp: 78F.

All of them apart from synspilum have it. I added quick cure mardel.

How could ich entered this tank? What else should I do?

Just had it in 2 of my tanks. Raised temp to 86, daily gravel vac with water changes using aquarium salt. Cured them in less than a week. Bought medicine but didn't even have to use it.

I don't like mixing medication and heat. It tends to stress out the fish too much. Just follow the directions on the medication and do a gravel vacuum, adding back in the medication removed when you do the water change.

After the medication is complete, do a couple of 75% water changes in a day to remove the medication. Then raise the temp by a couple of degrees every 4 hours until you get to 86F and keep it there for the next couple of weeks. Do a gravel vacuum of the substrate every 3-4 days during the raised temp period.
 
Do you use feeder fish, or have you added any new plants, or driftwood?
Ich protozoa have a dormant stage, and although it is often thought to be short, it is possible certain dormant cysts can be dried, or frozen and reappear/emerge after years in dormancy.
 
How could ich get in this tank though!, no new fish or anything it can just randomly come out?

Check your posts: Chocolate was added to the tank on 3/9 or 3/10. Started seeing ich on 3/20. Ich doesn't have a dormant stage, although at really low temps it can take weeks to emerge. It was either attached to the Chocolate or simply free swimming in the water you got from the LFS. At the low end of tropical temps, it could easily take 8-10 days to notice. (e.g., life cycle of 7 days at 77F.)

If you can't quarantine a new fish, it's a risk you'll need to take unless you institute some preventative measures.
