Well,for starters that fish is not an Oscar,it looks very much like a Jack Dempsey and that patch might be some type of bacterial or fungal infection or possibly a heater burn?
Oscar is the name of the fish and I agree with the Jack Dempsey ID and possible problems. Water parameters would be helpful when you get them as well as water change frequency and any tank mates.
He's an "old school" keeper, still haven't convinced him to condition his water, his routine is sporadic at best. Honestly you would not believe how successful he is with this setup has been keeping fish into there teens this way for over 40 years.

Does look like a jack, though he looks a bit deformed. I would agree the white patch is probably a pocket of puss underneath the scales due to an infection of some kind. I would probably go straight for something like metronidazole, usually I would recommend a more mild medication first, but this looks pretty severe.
