INTERNAL PARASITE EXPERTS PLEASE HELP

I'm real confused right now. My ph is perfect,my ammonia and nitrite are 0,my nitrate are very low. I have a 500 gallon tank and i'm running 4,000 gallons per hour. A week ago I seen something that looked like little tiny maggots stuck to my glass but they dissappeared within a few days but i've lost a few fish within the last month or two which is weird because all of my parameters are perfect but now that I tink about it all of them had similar deaths.They wouldn't eat for a week or two and then all of a sudden they died.Could I have internal parasites in my tank?? All of my fish that died looked like they were in perfect health. If someone on here has ever seen anything like this please let me know how to treat.
 
search planaria. feed less, water changes more frequently. planaria would not kill the fish
