Looks like the start of HITH (hole in the head disease).
This is usually brought on by some sort stress.
The most common stressor in aquariums is high nitrate, due to a lack of enough, and large enough water changes.
Without knowing the set up, water change schedule, and nitrate concentration, it'd be hard to determine. cause
