If no pleco in the tank, then it could be those candy looking sharp gravel. It probably tried to bury itself but in the process, it scratched the head. Those gravel will not be safe once the bichirs grows bigger. Get a grain size fine gravel or best yet, sand.
The water stats is good and there is a royal pleco in the tank. The 2 endli is 4in and currently in a 30gal, but soon will be in a 100gal once he reached 6in. I really dont think pleco is the case because he's always stick on the driftwood or glass. If pleco sucked on my endli, i would c those on the body too. I guess it might be the gravel like King-eL said. Will they heal or i have to put in medicine? Thx for the help guys.

Those feeders were bought yesterday. Both endli were on pellet for months, so i decided to feed them a couple of feeder for a days. Their head were like that before i put in feeders.
But goldfish are like ...icky? and nasty little fish, putting them in with a sick fish wont help at all.
If you want to feed them fish, go get a chunk or raw tilapia, I did the other night and thats the first time I ever saw my Del eat in front of me, she loved it!
