Is this hole in head? He’s the only one with any signs in the tank. Parameters are good, had an ammonia spike a couple months ago after emergency tank switch but used prime. Thank you for any info and treatment

The whitish marks do look like HITH. What are you nitrate levels using a liquid test? How often and how much water do you change a week?

My levels are 0,0,10ppm. The only changes other than the tank a few months ago is I added an additional c360 canister filter so now I’m running two in a 125. Should I start treating now?

Also, I’m changing 25% every 10 days or so. Have done anything about a 30% in a long time

New Life Spectrum hex shield would be the best treatment. You will probably have to starve him for a couple weeks before he eats it.
