My largest Efasciatus (about 8 inches) developed what looked like a white pimple. At first I thought it might be a wound of some kind.

But now it seems to be spreading, so probably an infection or parasite of some kind. Is this Ich?

His behaviour is normal, and no other fish seem to be affected.

Looks Bacterial but not Ich.

I agree it could be Lymph. and because its viral, the only really viable treatment is a low stress environment with lots of water changes.
The stress can come from being shipped and new, or another fish in the tank harassing it, or high nitrates, all stressors that could supress the immune system

It might actually be a few small scrapes in which a fungus has taken root on the damaged tissue. Or then again it might be lymphocystis. Either way, clean water, good food and less stress is the best treatment.

I am leaning with bacterial or viral. clean water and can try treating with kanaplex. if viral nothing really for it cept as mentioned.

If in fact it is Lymphocystis, then I would quarantine the fish. About 8 years ago I got a shipment of fish with it and it spread and wiped out some fish.

Smitty, I remember when that case of lymph hit your tank, must have been the most stubborn and virulent case I'd ever heard of
Sometimes all it takes is one day in a box on a plane, to shatter a fishes immune system.
