About a month ago my J.D. was showing the symptoms of having parasites.
He would scrape against the drift wood, shake his head, and wouldn't eat.
I took the carbon from the filter and gave him medicine for internal parasites.
He started acting a little better after treatment, but still would not eat.
Since then I have given him treatments 3 times using different products for parasites and he still shakes his head, and refuses to eat anything but blood worms.

It's definitely not ick, There is nothing outwardly wrong with the appearance of the fish.
He shakes his head like he has epilepsy, and doesn't eat. Some of the meds I used were for internal, and external parasites. At one point I did treat with malachite green, and raised the temp in the tank, and this had no effect on the fish. I have done massive water changes, and have been keeping a close eye on water parameters. He was a greedy pig, and now he doesn't eat. He will eat a few blood worms every three days or so.

What meds did you use and what are the active ingredients in them exactly?
What is the appearance of its poop?
Water parameters exactly?
Tank size?
Temperature?
Tank mates?
Filtration?

Until you are sure of the actual culprit of this issue, I suggest against using and switching the meds too fast. You could end up killing your fish than actually help it.

Jungle fungal for internal, and external, and anti parasite medicated food for internal parasites, also tried malachite green. this has been going on for about a month. I haven't really seen him poop, he isn't eating. PH is between 7.2 and 7.6, ammonia 0, nitrite 0.
He is in a 50 gallon tank. temperature is eighty degrees. Tank mates are a five inch Chocolate Pleco, and a two inch female Pink Convict. The Jack Dempsey is around six inches. Filtration is AC 110. media is lava rock, carbon, and fiber fill.

Have you tried putting garlic on the food? Give it a shot and see if he goes for it.

What is your nitrate?
Do you remove the carbon when you treat?

I don't have a test for nitrate, so I don't know right now. I had the water tested at the fish store last week, and they said everything looked good. I always remove the carbon when I treat. I haven't tried the garlic, I will try that. Thanks, and if you can think of anything else it would be really appreciated. I got him when he was a baby, and have grown attached to him.
