Would appreciate any ideas here because I'm out of them.

My friend's 3 y/o JD has been sick last December. I'm surprised he's still alive. He originally was put into a hospital tank after a slime coat issue. He was all white and shedding. After the hospital tank period of a week, he has never been the same. He will not eat pellets at all whereas he would happily eat them before. He was shaking his head, spitting, and almost convulsing a few times a day especially at feeding time. I recommended parasite treatment of metro and prazi. 10 days of metro in food, and 18 days of 2x dose of prazi in 3 doses every 6 days with 82F water temp. This was a few weeks after he was treated with Kanamycin and nitrofurazone by his owner. I do believe it was a fluke issue as the other tank he had was also treated for scratching fish and has since been fine.

Edit: I'm reminded the fish was also treated with Clout back in March.

The Prazi and metro seems to have helped with the 'shaking' and spazzing out, but he still refuses to eat. My friend was feeding him crickets and bloodworms and occasionally shrimp - which he did eat a small amount after the metro/prazi. Now he is trying to get him back on a pellet diet but the fish has not touched them for over 3 weeks. We have tried soaking the pellets in Garlic Guard and blood worm juice w/o success. His stomach is severely concave. I am worried he doesn't have much time.

I know that fish won't usually starve themselves to death, but this fish may be an exception. When he was eating the crickets and bloodworms his poos were normal. My friend is about to give in and get him his crickets and bloodworms to keep him alive, but thought I would check here for any possible ideas first.
 
See if he will eat some live Brine.

Has anyone had a cichlid that refused to eat pellets even after 3 weeks that wasn't sick? We're holding out on the meat, but don't want him dying either.

I had an oscar that ate pellets till he was 6" and then he jumped out during a w/c, a week later he started getting picky on his food and only ate crickets. I threw in pellets and nothing, then I tried feeders and still nothing but when ever a cricket hit the surface it was gone.
 
I had an arowana that only ate crickets too. He wouldnt eat feeders or anything else. There was one time only when I ran out of crickets and he ate my rainbow shark.

Let me ask you this Andy02793: Were these 'cricket only' fish healthy? Did they live long lives? I had a EBJD that was raised on bloodworms before I got her. The fish did not live more than a couple years and seemed to develop swim bladder disease and died. I don't want the same for this fish.
 
I would do as smitty said and attempt to get him chasing some live brine shrimp. Dont put feeder fish in because they can cause disease in the tank.
