java fern randomly dying

I have a 40g discus tank with java fern, anubias, red ozelot, banana plant, and I had rotala, but it all died.

My java fern has shown great growth, and all of the sudden, it's starting to get brown splotches all over the leaves, until the leaf is completely brown.

The only changes I have made is putting in an ozelot sword and a banana plant as well as rotala, but it all unrooted and died. I also added root tabs and began dosing pfetz medium tech, which is just nitrogen and micros, as well as putting in a fresh batch of fermentation c02 (I don;t think it was working before I replaced it.

I'm awaiting a new shipment of plants, so I really hope that they don;t all die because something is out of balance!!

is the water hard in your area? what are your levels like? make sure you reference proper water conditions for your plants. this was my issue during my first planted tank
 
I'm not sure if the water in my area is hard, but I use a discus ph buffer and tap conditioner. I wouldn't know if my water is hard because i use an RODI unit for all my saltwater tanks, how do I tell?

And what do you mean when you say reference water conditions?
 
In my aquarium book there's a picture like that and it suggests iron deficiency. But I kill plants, too, so don't take my word for it.
 
Is it possible that there is too much iron? Because the root tabs have a lot of iron in them, and I just added them a week or two ago. I didn't even use the recommended dosage though, so I don;t know how that could be.

I increased lighting a month ago to make way for my new crypts, swords, sagg, and valls while previously I was just running flourescent, I now run a 2x39w T5ho fixture
 
bump, could my fixture be too much lighting for java's, or is it something else I recently changed like C02 or root tabs? Please answer asap, all my new plants came today and I really don't want them to die!!!
 
Why would you use root tabs on a plant thats roots are not suppose to be Buried?

It looks like the java is attached to the wood..is that correct?
