To start she was an amazing eater, eating shrimp, chopped night crawlers, diy gel food, and pellets. She was also super active.
about to weeks ago her appetite decreased a little daily till yesterday when she didn't eat, but shes still fairly active. Ive seen rays with head dents they're usually under fed. How ever this little girl is not under fed shes still a little plump. The only thing I can think of is she got a parasite from a earthworm? all her other tank mates are completely normal. she was the only one fed night crawlers. last night I moved her to quarantine and started a prazi treatment.
any other ideas? the water she came from tested well at

I've never heard of parasite issues with earthworms. Captive or wc?
 
Any pics? Trying to picture what this could look like. Its a recent development and not a birth defect?
 
Could be stress induced.

I thought I was gonna loose my Henleo here last week. I knew a male was harassing her, but misjudged how bad it was. She had a giant dent within about a day and a half of going off food..... Put into a tank by herself and she was eating again in 2 days, couldn't tell the dent was even there within 4-5 days.

Sounds to me like you took the right approach by seperating her. Hopefully appetite will return soon. I imagine the Prazi will only help with that.

^^Ditto. I have been told from certain importers who believe that stress can induce the forehead dent.
