Losing fish at wc time

Never had this trouble before. I do weekly waterchange and the past two weeks have lost fish each time. Within an hour of refilling I have a floater last week it was a pictus and a silver dollar this week another pictus and another silver dollar. These were basically dithers for my umbee afraid of losing umbee seems fine so far though.
 
I do a gravel vac stirring up sand every other week and wash out filters on off weeks so as not to crash cycle was adding salt every wc but I was afraid salt may be culprit so o didn't add this week. Only thing I add at wc time is water cond. API water conditioner super strength
 
The wcs are about 70% a little more than normally suggested but I've never had an issue with it. I gravel vac then refill with a garden hose connected to a utility tub in basement. Hose is only used for water changes. I monitor temp with thermometer and keep within 3 degrees upon refill.
 
I understock, overfilter, feed conservatively and change water religiously haven't lost a fish to an indeterminate cause in over 8 years could I be changing too much water? Idk
 
I think your problem is too big of a water change and use of the hose. I'm assuming you refill the tank with tap water then add conditioner...right? That'll do it. I don't use any kind of conditioner but with that much water out, I would or use aged water. I would treat the water before adding it to the tank
 
Remaining fish are out and about begging for food alread. The ones that died just floated to the top like immediately they kick a few times upsidedown spiraling around the tank and are stone dead within 20 minutes.
 
Ic that makes sense I will cut back to 30% weekly from now on. Is there anything I can do to minimize losses now. Normally I would change more water to dilute whatever the issue is but obviously that would be counterproductive with this
 
I typically add the water conditioner at about 50% full but other than that your description is spot on
