It dosen't look good, but I'm not a goldfish expert, I can try to help but right now i'm as puzzled by the situation as you are!
 
add salt first and see if it goes away. 1 teaspoon every 5 gallons also up the temp to about 83

Liz, usually, a bump or lump that suddenly appears, is an infection particularly a bacterial infection. In this case, the fish must be isolated immediately and away from other fish that may not be infected yet. The pus may burst itself like a boil but do not attempt to squeeze them yourself as it is fatal to fish. It is difficult to say which of the two bacterial infections may be involved, columnaris or aeromonas although both are gram-negative bacteria. A combination of Maracyn and Maracyn 2 may help prevent this or tetracycline if your pH is no higher than 7.6.

Yes, a heater should be added, Liz. As for salt, I'd use it if the pus burst.
