Melon Sword Problem

I have had this melon sword for a month now but all of its new leaves stems die. It just got a new one the other day and the middle part of the stem died but the bottom of the stem kept growing. I'm using sera root tabs as a fert and diffused diy CO2 (c02 is broken right now though) Any ideas?

How do I tell what substrate I have?
 
Oh and tank hight is about 19in and ph is around 7.8 kh I think it was 4
 
do you add any supplements to the tank at all, swords are pretty fast growers, but dont last that long without any type of special additives In My Experience. the lighting sounds good to me though, as they are very picky about lighting needs. they do fine in low light. what is the stock and how large is the tank, as a tank with no fish, tons of plants and no added supplements is not going to go very well, as there arent going to be a substantial amount of nutrients for the plants. maybe there could also be whats called an imbalance, when there is high light but no added ferts and Co2?
 
I Am giving them sera root tabs and sera liquid iron ferts also a potassium fert. Also I am doing DIY C02 and I diffuse that with a hagen elite mini filter
