MIDAS: Single Hole in the Head?!?!

Have had issues in the past with HITH with other fish, but never anything like this. 4 days ago I noticed a small pimple or scab on my Midas. Now today looks like a single large hole! He has no other marks on him except this hole. Weekly 30% water changes. 0/0/10 water. Fish is 7 years old or so and never any health problems. 150G tank shared with Melanurus and a few plecos. Fish is eating and acting normally.

Any advice or suggestions?

For a starter I would start upping the water changes, the more clean water you can throw at this fish the better. Add some salt to the tank and see how things go. Be ready to act with antibiotics if things get worse. Good luck.

Thanks. Have some plecos in there so must go easy on the salt. May remove them for awhile.
Have plenty of antibiotics: Kanamycin, nitrofurazone, and even some chloramphenicol should there be any sign of infection.
Metro dosed even internally w/ pipette has been totally ineffective with other fish in the past w any HITH issues.

I have never seen anyone beat a case of hith with meds

I have used metro force fed with a pipette in the past. Had decent results. NEVER have I had Metro used in the water do anything.

In this case, it seems like 50% WC every 4 days, less feeding, and 3 cups of salt in the 150G did the trick! Wound is not really even noticeable any longer. No antibiotics needed.
