Improper diet and overfeeding, obesity and dropeye.

Please feed your fish a varied and proper diet, for example arowana a are insect eaters, and can develop dropeye if their diet is to fatty.
Many cichlids eat primarily insects, only a very few such as peacock bass are real piscivores.

Don't over feed your fish either, your fish shouldn't have a bulging belly all the time, instead a slightly rounded (on most fish) belly is better.

Some fish benefit from exercise and enrichment objects, simply making them chase your hand (with food in it) can help your fish do better.

Lack of decor: years ago people thought that exhibits and cages that were bare and easy to clean would keep animals happy and healthy, however we now know that enriched habitats with more complex things like rocks, plants, toys and other objects help both a animals immune systems and reduce aggression and seem to ease a animals mental state.
This has been found true in many animals including fish, many of which have a learning ability greater than many "lower" primates.

Lack of tannins: many fish, especially s. And c. American fish benefit from tannins in the water, which boosters the immune system as well as working as a antibacterial, tannins are especially beneficial to many tetras.
Tannins can come from: driftwood, dead leaves (such as oak or almond), acorns, etc

Improper tank size:
Please give your fish tanks of the proper tank size, tanks should be wide enough for fish to easily turn around without touching anything, and at least 4 times as long as the fish.
Don't keep your pacu in 325 gallon, they need at least 1000 gal, same goes for redtail catfish.
Oscars shouldn't be kept in a 55 or 75 gal, instead a single oscar needs at least 125 gallons.

In to small of tanks Waste builds up quickly , harming your fish.
Spinal problems occur from not enough space, and abrasions occur more frequently , fish can also show maladapted behavior such as jumping out, hyper aggression , lethargy etc

Too much algae:
Algae in it self isn't usually harmful, it can be reduced by : doing more water changes, limiting the amount of light, getting fast growing plants to outcompete it.
Another method is to use algae eating animals such as snails, plecos, otos, algae eaters, hifin sharks, stonerollers, flying fox fish, etc
