In the past three days spots showed up on his head and now they appear to be pits. Thinking HITH, what is the best course of action. His attitude has totally changed and hardly eating. Also seems to have a sore above his left eye.

I would say do a 50% wc everdday or every other day for a week or two. Get some medicine. Someone else with more experience will chime in. And yes i think it is hith

Post info about your tank, parameters, filtration, tank mates, how long its been established etc. I would do a water change to start. Those pits look like HITH, but that sore could be a injury from rubbing or something.
Water is perfect ph 7.9 nitrite 0, running two ac 110's , 125 gallon, tankmates, 9 inch synspilum, 3 inch freddy, two 4 inch festae. No torn fins hardly any aggression. 30-40% water changes weekly.

No ammonia, he's been on massivore for a month or so, was eating like crazy. Could that be a factor?
