Is there any other cichlids in the tank or any anything being mean to it? The inside portion of the lip just looks like he got into a lip lock fight with another fish, the wound on the outside of the lip however looks like it probably came from the pleco's spine.

As just how to treat it I would recommend giving the tank a water change and dosing with aquarium salt. Afterwards I would just focus on keeping the water clean and the fish fed. If he doesn't eat right away don't panic, it may take his a few days or more to feel like eating again.

Thanks , so it doesn't look like a seriously infection or diseases right

Nah, I wouldn't worry too much about it atm. It looks pretty clean so I would just do as I said earlier and just watch for any signs of infection.
Just done again water change and add more aquarium salt , ratio 1table spoon per gal
Is that right

I believe so. It should say on the back what the dosage is. But yeah at this point it's just playing the waiting game till the injury heals. Knowing Oscars' it should make a full recovery with little to no long lasting affect. Might have a sore mouth for the next few days but that shouldn't slow it down any.
