She looks like she's getting picked on. What size tank is she in and what are her tankmates? What is your water change schedule like?

how big of a tank is she in? you deffinatly need some gravel and some caves or something for her to hide in, in a open tank like pictured if shes being targeted she has no where to go and im sure is quite stressed out.

That tank looks like a 29 gal and I can spot two female Jack Dempsey, a Pleco and an Oscar. You have no substrate, no hiding spots whatsoever, and not even a background. The water looks so cloudy you can almost smell the stink through the screen. She needs help indeed... your setup is the fish equivalent of a padded cell in a mental hospital.
