We're in the process of relocating state to state. Earlier, I had set up several tanks to cycle for the fish transfer. Everything went well, the tanks cycled perfectly so, we transferred the fish we kept in LA to the hatchery in MS. Among the tranferees was my pair of 2ft+ ornate polys. They transferred well and ate the day after being deposited in a 210 gal. We had to leave for the other house for several days so, I fed out all of the tanks a little heavily. I overfilter so, this has never been a problem.
The return hose coupling to the 210 came loose and drained over 1/2 the tank and, of course, ended all filtration/circulation. Going by the rate of decomposition, the mishap had to have occurred just after we left for our 4 day trip. The hose coming loose combined with a heavier waste amount from the heavy feeding fouled the tank enough to kill off the pair of ornates.
It wasn't until the disaster that I noticed that I forgot to install a 50 cent hose clamp on the return. I saw it missing when I started the tank up but, forgot all about it.
