There are many things that can cause this and by the time things reaches that stage it is often to late.
First, are there any other fish in the tank and if so, are they okay, if they are it is not toxins.
Check the temperature, adjust if needed.
How do you mean spazzing out?
What do the fishes feces look like and what color is the gill tissue?

Water does look bad. Just as a future note, you are not supposed to bury the heater. Give it an inch at least of space.

The fish did die eventually, I did remove him from the 120 and put him in my hospital tank with no luck.

The water you guys think is bad is actually just my camera, the water looks flawless in person.

I checked all my parameters and everything is normal. All of the other fish in the tank are absolutely fine.

If you read my other thread in the cichlid forum, there is a more detailed information. but in short, all those fish were moved yesturday, so im guessing it was the stress that killed him.

Thanks for your replies.

It would take a lot of stress to kill an oscar that size
