Need help! ASAP

My hbrtg is sitting on the bottom and has been twitching rather violently when it swims around. What should I do? Parameters are fine and all the other fish seem to be fine. No signs of ich. Water change and add salt? Melafix? Please help. I don't want to lose this guy! He's about 18" in a 230 gallon
 
Have you been feeding him live fishes?
If yes then it might hae gut infection.
Check its faeces, if they are different in any way.
Is he eating properly or not.

If gut infection is suspected, use gut clear medicine from oceans free. Its very effective.
and add salt and raise temp to 30 degrees celcius.( not farenheit)
 
Why do you say your water parameters are fine, and then you ask if you should do a water change ?

When was the last water change? Do you clean the gravel too ?
 
Last water change was 2 days ago. I was thinking fresher water is always better. There is only a thin layer of sand. so no I do not clean the substrate.
