Need help hole in the head

Hi I have a 55 gallon with a green terror that has hole in the head not eating and white stringy poop what is the best treatment for it ? Have a emperor 400 filter and temp at 80.5 and do a 50 percent water change every week and there is 5 clown loaches in the tank should I tank them out ?

Generally hole in the head occurs as a result of poor water conditions.

What are you readings for ammonia, nitrite, nitrate, and ph?
