Need Help with Disease ID (with pics)

I have a 5" or so Green Texas in a 165 Gallon with a female jag, mayan, synspilum (all of similar size), 2 bala sharks and 4 pictus cats.
A while back, I noticed a small red bump protruding from his side, kinda like a pimple or wort. I watched it closely and since it did not seem to improve, I took him out and treating him in a hospital tank for a week with aquarium salt, melafix, Maracyn, and Maracyn 2.
It seemed to show some improvement and since I was about to leave town, I put him back in the main tank (probably should have treated longer).
The original spot is now kinda a whtitish patch that still protrudes a little, but he has gotten other spots on him, around the base of one of his pectorals, one on his tail fin and mostly around his mouth. They seem to be mostly in the crack between his lips and head (where the mouth expands when they "yawn"). Recently, one size of his mouth has swollen out a bit. He is not acting any differently, and has actually started eating more than he was (and a little more outgoing/aggresive).
No other fish have shown any similar symptoms in the several weeks this has been around.
I think it is safe to rule out any type of injury from aggression due to the appearance and location of spots. I definately have not encountered anything like this before and it does not match any illness/disease diagnostic information I have come across. Definately not Ich or Bloat, which I have seen in the past. Seems more like a fungus, but does not match photos of mouth or body fugus that I have seen.
Does anyone have any ideas???
I think I'm going to have to separate and treat again, possibly adding metrodinazole and Jungle Parasite to the treatment as well to try to cover all bases.

I'm sure people will ask so I just tested the water. Parameters are:
Temp - 82 F A little high but it's been the mid to upper 90s for a week here in NC...
pH - 6.4 A little low, I always use neutral regulator at W/Cs, but I have some driftwood still leaching tanins that I think is also pushing pH down...
Ammonia - 0
Nitrite - 0
Nitrate - 20
 
Could have been a cyst. Few of my fish have gotten them. Never had a problem with them, they usually pop and heal. If "white" growth has appeared on it, could be columnaris. Keep a good eye on it. If the growth gets worse, treat with antibiotics.
