need help with my female severum

Need help with my female green severum,
i'm not sure whats wrog with her. she seems to be lethargic, and also her bars are all out. i was supposed to move her to a hospital tank but changed my mind. but i'm really uncomfortable,. feel there something wtrong with her.
any suggestions? she's the only one not looking well in my 300 gal tank. her pair is healthy and quite active. also saw her with her head raised up. the opposite of head standing? but when i came near she was in a normal position again.

should i move her to a 15 gal hospital tank?
 
How are the water params?

How old is she?

Ever showed signs of breeding?

Is the hospital tank set up and cycled? And over filtered?

If it is and she is moving around kinda well when you come around, cover the sides and back of the hospital tank with dark paper and give her something to hide behind then move her there for a week to watch her more closely w/o the stres of the other fish.

Monitor her food intake carefully and clean out any extra so she only eats when you feed her.

thanks, she's been with me for 2 year i think, but i already got her when she was an adult.
she used to lay eggs , but i didn't removed the fries so they ended up being eaten by other fish. I'm not sure about the nitrite /nitrate/ammonia level, but the tank should be cycled. since its been running for a few years now. other tankmates seem to be fine.

my hospital tank only has a sponge filter i removed from another established tank.
she seems to be developing hith :(

Planning to medicate with metronidazole. and also feed her some vitamins :)
