Need ID and help with this plant

Bought this plant a while ago from a lfs b/c I thought it looked awesome. The guy told me the name but I don't remember. The problem i'm having now is that it is starting to get brown and it's leaves are starting to tear, I need some help to try and keep it healthy. All of my other plants are doing fine.
 
Looks like Aponogeton ulvaceus to me.
Your issue is probably the gravel. These guys are heavy root feeders and need nutrients in the substrate to thrive. I would recommend picking up some root tablets to provide nutrients to the plant. I prefer the pFertz root tabs but there are many other options, for example RootMedic and Flourish. Try some tabs out and see if the plant improves in health or not. Aponogeton sp. are generally pretty hardy so you shouldn't need to upgrade your lighting or add CO2. Best of luck and keep us updated on it's progress.
 
Aponogetons are bulb plants. Like most bulb plants, they have a resting period. The leaves turn brown and eventually die off. I believe this is what you are experiencing. If the plant developed a good root system, it will in time grow back.
 
I don't think the problem is the gravel. I forgot to mention I have a layer underneath the gravel what had a mix of sand, soil, clay, ect. for the roots to grow in. I think it's most likely going through that resting period.
