Water parameters are spot on. New fish bought on Saturday from reputable LFS. Never had a problem with any fish from this place 90% of my fish are from this LFS. Stated four years old, four months at LFS. Stated they never noticed anything wrong with Gourami. Under lights at LFS I was unable to see the discoloration and pivots and holes in his head. Upon finishing acclamation with the lighting in my tank I was able to see the forestated. Zero ammonia zero nitrite, 20 nitrate. Soft water. Planted tank with two invertebrates. 55 gallon community tank ranging from black ghost knife, Corydora,angel, clown Loach,Rams, neons, glassfish, extra large/medium/and clown Plenco. I have read about jungle laps hole in the head guard.do not want to add any medication until I know the effects on my tank. If you have suggestions for me please let me know. She eats very well, frozens, tropical flake, Beefheart flake, and floating Betta food .she is swimming in the daytime sleeping at night, very Docile in the tank. By the way temp is at 80° .to add I do not believe this is a parasitic hole in the head. Because I do trust the establishment she was bought from. Also she was there for four months. She was tanked with a lavender Gourami only. But I have never had this issue before.

I found it on Amazon and I'm seeing that it's a sinking pallet he's so big in the tank that the Coreys in the loaches will eat it more than he will, do you know of a comparable floating pellet, or flake ?

Do u think it's probable; after looking at the pictures ,that "gourangi" Being four years old;may have had hole in the head previously, and was subsequently treated? Thus, Leaving the "scars" we see now? I don't see any current pimple like crustaceans.
As I look at the pictures closer as well as well as looking at her; it seems to look slightly different then pictures I am finding after a search request ,on google images, for hole in the head. I am currently buying Boyd vitacheam and API metafix. She now has a very diverse diet. But I did learn upon watching a YouTube video about hith, that my black ghost knife fish, needs a vitamin supplement since he only eats frozens. The introduction of both should also help gourangi, at least that's the plan. Please let me know about a comparable anti-parasitic, that comes in the form of a 1 mm floater, or a flake . Thank you for your input .
 
I thought you were talking about an active issue. If it's just old scarring don't worry abou it. I don't know of a floating pellet for hex, but you could soak some pellets yourself using Metro powder.
