i just bought this guy from the store i work at and he seems very skinny to me. are there any really good foods to fatten him up or could he have a parasite?
this is the best picture i could get as he never stops swimming on the glass.

They do digest very fast. You can add smaller sinking pellets like NLS Thera A or shrimp pellets. Some go after it right away but some wont touch it.Worth a shot.I also use smaller snails like MTS or Pond snails. Also small pieces of shrimp or krill and clams or other crustaceans work as well.Eventually crawfish are a welcomed treat but it may need to get some size on it first.

Just give it a very small amount of pellet and make sure it sees it.If it does not eat within 5 minutes, take the food out of the tank so it doesn't foul your water.Good Luck~they're awesome fish!snails always work also

shrimp and snails should help

just as much as any othe fish would. I wouldn't make it rapids conditions in there or anything but IMO more flow is usually good.

He might not go for the pellets, you could try, but fresh raw seafood is probably going to be something he will enjoy more.

You can try
shell on shrimp
prawns
Crushed crab legs (use a hammer to crush it a little (eating the shell with be good for his teeth)
Squid
Octopus
peas
Live earth worms/night crawlers/tout worms <-- BEST fatten up food for puffers
Cockles
Oysters
Snails are great as well, but your fahaka will quickly outgrow ramshorn and pond snails, when that happens feed other shelled foods

Here is an article on how to feed your puffer!!

Feed him some live freshwater crabs (red clawed or some of the other some ones near that size), live crayfish, night crawlers (puffers love live wiggling worms), some krill, clams, and any other meaty food. Most puffer wont even eat pellets. Mine wants anything meaty or live but no live feeder fish (be safe then sorry). Purple Candle has a really good food menu for Fahaka's. I just added a little extra.
