One of my convicts has holes in his face!

So, like the title says, one of my convicts has holes in his face, and I don't know if this a disease or if it's just a defect or something. I feel like it's been this way for a while, almost ever since I got it from petsmart. None of the other fish in the tank have the holes, and this guy doesn't really seem affected by it at all. It's like four holes on his face that look almost like nostrils. He also has a dent above each gill kind of.
 
its called hole in the head disease (HITH) usually a result of poor water quality
 
Need a picture. There are many natural and normal pores and pits, and there is HITH as described above. HITH is generally ragged in shape and erodes away the tissue. Sensory pits look like pores and don't really change shape.

Here you go guys. Sorry about quality, but its the best I got with my cell phone camera. I think it is hole in the head and that would make sense being it came from petsmart like that. Is there anything I can do to fix it or is it always going to be like that? My params are always 0 ammonia 0 nitrite 10-20 nitrate, and I try to overfilter my tanks.

The pics is kinda fuzzy but I am leaning towards hole in the head. Make sure your water is clean and sufficiently filtered \ changed.
 
Yeah it's HITH unfortunately. Even though people say it's caused by poor water quality, it isn't always. I had a blood parrot who got it completely from stress because he was always fighting with another male, and both were trying to be the alpha male of the tank. It was a very large tank with excellent water quality, though. Once they were separated it resolved within a month.

Others think it's caused by Hexamita parasites; there are many theories. If everything is going well in your tank, and he's had it since being at Petsmart, I think he could have a compromised immune system. I've known of a few inbred fish getting it for no obvious reason.

This is true, and why I don't buy fish from chain stores like that. The inbred fish have weaker immune systems. Some people say fish can inbreed w/ out any physical effects, but the thing is the damage may not always be a deformed fin or tail.
The HITH can be cured though. I did it buy raising the water temp and using aquarium salt.
