Hi I have had my red oscar for just over a year now and he was doing great and still is, but every few months he starts twitching and he just did it again last night but has stopped since I went to bed. I'm not sure if it is a medical issue or just something they do. I have a video of it twitching if it is needed.

Maybe he sees his reflection at night when the tank lights are on, so he’s displaying to what he thinks is another oscar

I suppose that is possible but he has done it during the day too. He usually stops after an hour or so though.

if the tank light is on over the tank during the day, they can still see a reflection inside the tank
That could very likely be what is happening. My Bearded Dragon see's himself in the reflection of the glass all the time (he is usually kept outdoors in a chicken wire 400+ g cage, fyi). What are the tank mates?

2 bichirs 1 tire track eel 1 paratilapia polleni 1 frontosa 3 blood parrots 1 convict
all are under 5 inches except one bichir (endlichiri) and eel which are both 10 inches

Twitching is a behavior oscars, and all cichlids do to assert dominance and territoriality, and also an indicator that it could be mature and ready to breed. A female with developing eggs, a male hot to trot.
Is there a certain other fish in the tank, that the action might be aimed at? or just general in a certain area?
How big is the tank? And what are the tank mates?
