planted tank crashing

I have been having a problem with my 90 gallon planted tank. It is going to be used for discus (not for a while). The problem is the plants are slowly dying off. Got T-5 lighting H.O. 2-10000K that are on 12 hours a day and a very large peace of drift wood. The substrate is some natural gravel that is very small (60+lb) and 15lb of floramax. I pulled out a clump of mondo grass and the roots are rotting. What is going on. Last thing I have well water the Ph is 7.8ish out of the tap and very hard with lime and calcium. The test kit for hardness said 200+.
 
Probably because mondo grass (Ophiopogon japonicus) is a terrestrial plant.

algae on 3 sides of the aquarium. I liked the look. Was hoping to get a 3 walls of lush green algae surrounding plants. I am going to get a pleco (royal or hopeful a gold nugget). My other thought is to get more floramax.

For the picture sorry none. I am currently taking the tank apart, cleaning the drift wood that has black algae and re-aquascaping. The jungle val is turning black in some spots and getting some holes.

The bio load is very low. 2 breeding jewels 2 or 3 peacock eels (had sum jumpers), 4 inch black ghost knife and a flagtail. That was a short time ago. The jewels breed and the only thing left are jewels and eels.
 
Well sometime alot of algae can be do too high nitrates which is what would explain ur plants dying!! Test amd change water as neccesary!
