Please help. Is that the HITH or some kind of fungus?

What kind of treatment should i use?

Any suggestion ?

Definitely does not look like HITH.
 
Sorry but I can not really tell what it is. But with the picture as it is I can see that he does not have the lesions and craters of HITH.

PH, GH, ammonia, nitrites, and nitrate readings

You could try a 30 minutes methylene blue bath for 1 week, everyday. Mix 1ml of methylene blue per 1 gallon of water, and add about 1 tablespoon of salt per 5 gallons in a separate container/tank. Place the fish in the bath for 30 minutes with an airstone, and make a new bath everyday. I would use your old tank water to make the bath.

I need to get another testing kit to find out the PH, GH and Nitrate reading.
 
I put her into the hospital tank with the air temp and set the temp to 80 F.
She is not eating for a week or two.
I used the paraguard for a daily bath for a week.
Now she is having a little hole on her forehead and bloating + white string poop.

I stopped the paraguard and treated her with the metroplex.
Daily 15 to 20 % water change with prime and epsom salt.

Please help. Any suggestion? I really want to safe her.
She got special meaning to me. Really don't want to lose her.

Also sometime she will stay in the bottom corner or the tank or stay on top of the water but breathing normally. I covered up the tank so no bright light to the tank.
 
before moving her did she have other fish on her side of the divider? Does look like it could be erosion
