please help my green severum

I have one adult male Green Severum (about 6-7 inches) in a 46 gallon bow-front tank with some other community fish.

The day before yesterday, I noticed him not very active. I checked carefully and found one small region on his body with harsh scales and grey color, looks like the slime coating has been totally scratched off. He still eats but not very active.

Yesterday morning, I found that small region on his body extended a lot! I didn't know what the problem is. I checked the water and all the parameters were fine. But I still did 50% water change and added aquarium salt (1 teaspoon per 5 gallon). I was not sure whether this would help or not. I took couple pics and fed him, he stopped eating. :(

This severum has inhabited in this tank for more than two years without any problem. I feed him flake+shrimp pellet with occasional frozen blood worms. I do weekly 30% water change. All the water parameters are stable and good.

pH 6.8, [ammonia]=0, [nitrite]=0, [nitrate]=20 ppm.

Last night, I seperated him in a 15 gallon hospital tank and added melafix. I checked this morning, he was still surffering the pain.

Please help! Should I use some medication? If yes, what med. I should use? Thanks in advance!
