Pleco with possible bloat and tail issues

Hello MFK! We recently received a message to the MFK Facebook page from a concerned pleco owner named Janet. Her pleco has belly issues and tail issues, but well, I suppose I'll let her tell the story:

hello hope you can help me assess before we treat and treat wrong. we just noticed the tail looking like this and the belly bloated as we were trying to figure out why our about 15-18 year pleco hept swimming to the top fast and hitting the top of the tank, we have a 55 galon tank and 2 T-barb in there and the pleco. his eyes are a little cloudy and he is breathing fast. is there hope? ********

Bacterial: Inactivity, loss of color, frayed fins, bloated body, cloudy eyes, open sores, abscesses, red streaks throughout body, reddening or inflammation of the skin, fins or internal organs, bulging eyes, difficulty breathing.

Fungal (often secondary to another type of illness): Erratic swimming, darting, scratching, visible cotton-like tufts on skin, eyes, or mouth. Parasitic: Inactivity, loss of appetite, excess mucus or film on body, visible spots or worms, rapid breathing, scratching.

Do a 50% wc and add some epsome salt and metronidazole. Do not feed. Do another wc after day 3 and redose Epsom salt and metronidazole. If bloat decrease then try feeding small amounts. Repeat in another 3 days and so forth. Salt is 1 tablespoon per gallon.
 
To me it looks bacterial a round of metro would not hurt. I would focus on increasing water changes/ filtration and improving diet.

Also what has this pleco been being fed? I would stop feeding for now

Looking at first pic, it doesn't look too bad but the second pic, that looks pretty advanced.

do a 50% water change for starters then wait a few days then do 25% water change every other day for a week then see how it is doing
