When you vacuum hold the end of the tube .25-.5 inches above the sand.

Once a month after I vacum the poop from the surface I gently stirr the sand to release anything that maybe traps

+1 to above. I also keep a large colony of Malaysian trumpet snails to aid in mixing. They hide in the sand and are rarely seen except at night or after feeding.
