puffed then died?!

Ok, so my dog face puffer is doing fine. A bit shy in my tank but eating and swimming about frequently enough to look normal. I come home and he's puffed up with water because he's at equal bouyancy, but it never deflates. It was swimming around and then after a few moments, died. Is that a normal thing? Perhaps it could not deflate the water?

My tank is brackish but at 1.016 currently (the same that was at the pet store where I purchased it.) All other parameters look normal. It just ate some store purchased shrimp.
 
a puffer should never inflate in a aquarium, it means somethings wrong.

i dont know alot about dogface puffers but i always thought they were full salt fish with a salinity of 1.020-1.025

According to online resources Dog face puffers are one of the few puffers that live in and can tolerate a diverse range of salinities ranging from 1.010 (or lower) to full strength. I read that they actually do better in an environment that mimics the change in salinity (euryhaline).

I didn't do any tests, but the water is changed 20% weekly. I did just add two FW Moray eels that I rescued from a FW tank and quarantined and acclimated them in another tank. Other than that it was the time to actually DO a water change so I can't say if some parameter spiked off the chart.

The only witness I have is my 5 year old who called me excited saying the puffer was puffed! When I got home she said that my target fish (terapon jarbua) were swimming around the puffer, but she couldn't say if they were harrassing it or not.

I've seen some really jumpy one that do it if you catch them offguard walking up to the tank.
