First thing to ask yourself is it the same species as the rest? There are many types of red bellied piranha. My first shol of reds which consisted of NINE fish . . . which two as they grew exibited different character traits than the others. I say that because it sounds like both eyes of the fish are smaller and not just one, and its likely that it injured both eyes at the same time hard to do because they are on opposite sides of the fiishes head. But if it is indeed just one eye where the puple is noticably smaller most chances are ... it was injured either from a rock or tank decour or another fish while feeding.
