random minnow deaths

Hey guys, lately maybe once every two days i will find one of my fathead minnows (lake caught) dead on the bottum of the tank belly side up. i had problems with my water early this year due to increased ammonia in the tapwater but i've since cleared that up. my water Ammonia:0 Nitrite: 0 Nitrate: 60. i have a heavily planted 55 gallon tank with lots of drift wood and rocks. i've got about 6 different types of species of minnows and 4 small yellow perch, all fish in the tank are few different lakes. problem is every couple of days a fish dies. i have alot of oxygen in the water, i have a top fin air pump 8000 (2 of the air outlets are in use on both sides of the tank) a Fluval 305 and a aquaclear 70. (overfiltered because i have quite many fish in the tank). some of the fish are itching one sides of their bodies. i havent seen any white spots indicating ich but the tank is room temperature. maybe thats why the deaths are slow and random. i have many fish that show no symptoms at all which makes me beleive its not a quality problem. the dead fish usually look healthy but have one or two red spots around their gills.
the fish are cold water right now. do you think i should increase heat and treat for ich?? any advice will be appreciated!!

(i feed them twice a day, flakes and frozen brine shrimp and blood worms, dried bloodworms, water change is twice a week at 25% with prime and stabilize)
 
maybe 6 fish are lethargic and hide under a corner piece of driftwood, i havent noticed any fin rot or skin falling off.. maybe this could be ich , velvet or some type of parasite ?
 
I use these as feeders in my pond for my gar. In all honosty when I qt them ur gna have a few that are going to die off. There not very strong unless you acclimate them very well. Since I use them as feeders I can only use there very healthy strong ones so I barley acclimate them and put them in the qt and throw out or feed the dead ones to my crayfish.
 
well all the minnows i have are lake caught, when i collected them i picked only the most healthy looking. some of the healthier ones i've caught have died, they have been in the aquarium for 3 weeks now, each of the past three days has had 1 dead fish each night. im more worried about them scratching because i know there is a problem. i just need to figure out a diagnosis, so far i think its ich
