Rapidly producing high Ammonia Levels

My tank is rapidly producing high ammonia levels. I preformed a 50% water change yesterday after the water settled I tested and got some where between a 2.4 and 3.7. I treated the water with ammonia neutralizer and 30 minuets later tested again and got a 0. Today I tested again and got 3.7. My puffer is in a 55 gallon tank alone, with two filters made for a 75 gallon tank they are 265 gph a piece. There is fresh carbon in them...what is happening? I also rotated the sand when I did the 50% change. So I don't have any fears of methane build up either. I use the net and scoop out any left behind food and any feces he may have dropped. I'm at a loss here. The water is crystal clear and my puffer is acting fine, his coloring is normal and eating normally.

That was y first thought, but after I do a treatment it reads 0 or 0.1.
 
I'm using Nutrafin Ammonia Test it's a liquid test.this is the test I just did after treating with neutralizer. It's reading at a 0.1 now so I'll add a little more here soon.

The tank has been running since September. He had a 30 Gallon he was in prior. I had a beta and a few barbs in the tank prior it cycled with them for about a month.
 
I do a 50% change weekly. I pull his decorations and flip the sand during these changes and then within an hour I start reading extremely high ammonia. The only thing I can think to do is to put him in a QT tank strip this one and start over.
 
I've decided to drain the tank down about 80% I'm going to clean out the filters replace the media, but leave the bio stuff. Then I'm going to buy bottled bacteria and relaunch the cycle. The tanks been running about 3 months with no issues, so I don't know what happened, but I think this is the best option.
