really need help , hexamita!

I think my jack has hole in the head , he has a large pink raised bump on his head with very small white, tiny things proturding out of it. he is acting normally and eating but will turn upside down and rub the bump on decor and the gravel.

it appeared overnight after a 15% water change

Should I just keep the water clean or do I need to medicate.
