Can anyone tell me why my baby Red rails gills keep open? Water parameters are fine and he’s alone with a pleco in a 100galon tank?

Please list your parameters asap something is definitely wrong. Add extra oxygen with a air stone.

He has a airstone, as well as 2 hang on filters to ensure cycling of the tank. Also used reverse osmosis water , so PH is 7, its been like this since he ate too big a piece of food last week, (know it sounds silly, but hope its not to do with that)
 
Maybe it got stuck. It can happen. Think it may need to be seen by a vet if it’s affordable. Other option is to sedate with clove oil and examine the fish. Once again this is based on there is something stuck.

I didn't think a piece of food stuck would be that big of a deal if it's been there a week, he'd have died from it or he would have eaten/digested it. Has the fish eaten since then? if so then I don't think it would be stuck. I think your best bet, like kno4te said would be to take it to a vet or sedate it and examine if yourself
 
Ok will add more air, and see, as long as he’s ok I don’t mind if he stays like that. (He just needs to be healthy) yes he’s quite the eater when it comes to worms, but he doesn’t eat anything else.
 
This is strange. I've never encountered this in fish that are still live and seem well. Please, as requested, provide numbers, not words, for your ammonia and nitrite tests (which should be measured by a liquid test kit, not sticks), pH and temperature and their stability, and water hardness.

Perhaps Dave's hunch is correct and this is genetic. It'd be a first for me.

This cannot be from feeding, I can't imagine.
 
genetics IMO... u can clearly see its missing its gill plates... very commin in rtc, rtcxtsn and tsn farmies.... ive had cats missing 1 or 2 gill plates live for many yrs no prob... for the $5 this cat is worth... maybe worth getting another 1 in better condition if u have the means to keep 1 long term.
