Has anyone ever made a fatal decision with ONE fish and decided NEVER to do it with any other fish? Or decided that it was an isolated incident and kept doing it with their other fish?

Example: Water Changes...adding dechlorinator before, during, or after. Some have had fish die and changed when they add their dechlorinator or keep the same routine because it was an isolated incident..

Example: Feeding Live Feeders...disease getting their fish sick or die. Some have had fatal mistakes with this. Some now quarantine, raise their own Feeders or still buys Feeders because it was just an isolated incident.
 
My TSN fluffy.. ate an aba aba almost 2x his size... Fluffy now only gets tank-mates that I don't mind if he eats. no exceptions.

50%+ WC's .. don't happen in my house. I've lost to many fish over the years after doing more then that. Though some people swear by 50%-80% WCs. I've lost fish on more then one isolated incident after one of these to do them anymore >.<
 
your tanks are small. if you had larger volumes and true MONSTERS you'd understand the amount of waste and the 50+ water changes.

Feeding with live blackworms. I used to use them as a treat until it introduced septicemia into my tanks. Cost me some of my prize fish (9 SA cichlids and a ray) and cost me just under $1,000 to treat three tanks. NEVER again. I'm still irate about it to this day... Tough lesson learned.
